New electronic patient complaint card

Communicating your health concerns with your doctor just got easier. In a few steps, you can now describe your symptoms using our media enhanced electronic complaint card. We have a list of symptoms you can choose from per organ system, all you need to do is select your symptoms from this list and the app will organize them into your card for your doctor to review. You will be able to upload pictures and videos to your card in order to provide additional information to your doctor which paints a perfect picture of what you are experiencing.
